Baptisms at New Jerusalam (Swedenborgian), Shuttle Street
in the Parish of Tyldesley
Not much information could be found out about the New Jerusalem church except for the following entries within the Baptism Register.
CERTIFICATE or STATEMENT, to accompany Register Books, as recommended in the Circular.
To the Commissioners for inquiring into the state, Custody and Authenticity of Non-parochial Registers.
The annexed or accompanying Book is (or are) the original Register Book of which have been kept for the Chapel or Meeting house called - the register sheet containing the Baptism that as been Baptised being of the New Jerusalem denomination, situated in Shuttle Street Tyldesley in the county of Lancaster founded about the year 1821.
The Books sent has been from time to time in the custody of the societys meeting Room for the time being, of the Congregation: and is (or are) sent to the Commissioners from the immediate custody of the leading members of the society in the above place of who has kept it (or them) since the commencement.

REGISTER COMMISSION – Questions referred to in the Circular Letter
1 Are you or have you been Minister, Trustee or Member or Representative of any Minister, Trustee or Member of any and what Chapel respecting the Register of which you can give information; or in any way have you connected with any such? State the name, denomination and situation of such Chapel.
We have been the leading members of the society since the commencement and have been supplied by the Manchester Committee of the New Jerusalem conference at times with ministers.
2 Is the Chapel or the Congregation subsisting ? or if dissolved, when dissolved ?

3 Has any Register Book or Books of Births, Baptism, Deaths or Burials, (or of Marriages before the Marriage Act of 1753) been kept by the Minister or Trustee or other Church Officer of such Chapel, or in connexion with it?
No.
4 State over which period such registers extended. How many are there?
They are all in the inclosed sheet which as been sent by order of Conference for the purpose and to be returned when full.
5 Who is the Minister, who the principal acting Trustees, Deacons, or other Church Official of the Chapel with which you are connected?
The Meeting room is the property of James Mort the leading member of this society connected and acting from the dictation of Conference of the New Jerusalem church.
6 In whose custody is such Register Book or Books; and where is it deposited? In whose custody has it usually been kept, and where has it been usually deposited?
In the custody of the society library in the meeting room under the care of the leading members
7 If you know of any Register being lost state the time and occasion of the loss to the best of your information.
Being how lost.
8 Will you, or will the party having the custody of any Register, produce it or send it to the Commissioners? By what mode of conveyance will be used? If you do not produce or send it, state the reason.
What we have we have sent inclosed in the belongings to our registry.
9 If you know of any Register, which is not in your possession, give an account of it, as far as you are able, where it is, and what is the name and address of the person who has it.

10 May any Register sent by you to the Office of the Commissioners, remain in the custody of the Board for the purposes mentioned in the Circular Letter? If you have any objections to its being deposited, state it.
